Appointments will be for up to 30 minutes.
AN APPOINTMENT MUST BE MADE, NO WALK-INS. 

7 Items Needed to Complete Registration
1) Physical on a Kentucky form
2) Immunization Certificate: Accurate & Up-To-Date
3) State Issued Birth Certificate
4) Social Security Card
5) Eye Exam
6) Dental Exam
7) Proof of Residence (example: utility bill)

Family Health Care will be providing Physicals and Immunizations at NO CHARGE to you.
Appointments are required.
