Flu Clinic at Hancock County High School

Friday, November 9th from 9am-11am - Nurse's Office.

Students need to complete form to obtain Flu Shot - only page 1 needs to be filled out, page 2 is for Fred's Pharmacy documentation.  Student’s forms turned in by Tuesday, November 7th

Staff can complete the form ahead of time and just present the day of the clinic.
